The Association of Community Affiliated Plans (ACAP) is made up of 29 health plans, located in 15 states, which are primarily focused on the Medicaid, Medicare, SCHIP populations. Together they cover over 4 million Medicare and Medicaid lives.  Amongst the ACAP health plans, 12  Special Needs Plans (SNPs) are included. ACAP’s mission is to improve the health of vulnerable populations through the support of Medicaid-focused community affiliated health plans committed to these populations and the providers who serve them.
