INSPIRIS offers a different approach to healthcare delivers to the chronically ill and medically complex. – by providing a provider –driven care and care management solution based on accountability for outcomes and a proactive approach to bridging the gap in healthcare delivery. Our services include:
House Calls - an in-home care management program that targets members who are at immediate risk of acute inpatient admissions or emergency care – which lead to significant quality and cost of care issues. "In-home" can refer to any place the member resides, be it their residence, skilled nursing facility, nursing home or other institutional environment.
Patient Assessment Program – Full clinical assessments can be completed for those members in their place of residence. This assessment is designed to begin the coordinated care plans, ensure the proper care is provided, the proper diagnostic codes are gathered and the member’s needs are being met.
Transition Program - Designed to deliver short term care and care management this program will assist in the clinical needs of those members that are transitioing between a hospital, nursing home or place of residence.
