Grant Funds Available
Grant money is now available
from CHPW to help fund member community health center pilot projects targeted
at improving preventive health service delivery rates.
Our program goals are to:
¨
Help
support appointment outreach efforts (direct or indirect)
¨
Develop
a best practice toolkit to provide
tested interventions to other health centers
An example of a direct activity might be to send birthday card reminders to women,
encouraging preventive health services.
An indirect activity might be to
hire a temporary employee to enter
historical data into the CHILD Profile immunization registry for tracking
purposes & later use by CHILD Profile in generating reminder notices to
families. Other intervention examples might be health fairs, incentive gifts, or drawings.
We invite your participation
in this program & encourage creativity, as we would like to support a
variety of interventions for various issues.
While this program’s stated
focus is upon preventive health, we are also interested in supporting outreach
to chronically ill populations.
So, please put on your
thinking caps, & let us know what
service area you would like to target for improvement & what intervention
you would like us to consider funding.
The program application
process involves two steps:
¨
Completion
of a baseline survey of current preventive health outreach practices.
¨
Completion
of an application form detailing your project proposal, including measurement
plans for before & after monitoring to gage the effectiveness of your
intervention.
CHPW Medical Management will
then review your proposal, seek any additional needed information from you,
& provide a prompt response.
In addition to funding
support for approved interventions, the CHPW QI Team will be available to
support your efforts through general collaboration & assistance in defining meaningful
measurements.
After the project pilot
period ends (at the end of 2001), we will share your successes with the network
through distribution of the best practice toolkit.
